炎症性腸疾患に合併する腸管感染症の診断と治療

Diagnosis and Treatment of Enteric Infections Complicating Inflammatory Bowel Disease

要旨　ステロイドホルモンやインフリキシマブなどの免疫抑制治療を行っている炎症性腸疾患（IBD）患者では感染症合併のリスクが高い．実際，IBD患者において，腸管感染症を合併すると，複雑な病態を示し診断に難渋するとともに，再燃，重症化，難治化の要因となり，しばしば問題となっている．IBDに合併する主な腸管感染症としては，サイトメガロウイルス，ノロウイルス，Clostridium difficile，サルモネラ，カンピロバクター，赤痢アメーバ，ランブル鞭毛虫，糞線虫，クリプトコッカスなどが挙げられる．臨床所見や内視鏡所見から積極的に腸管感染症合併を疑い，それぞれに応じた適切な診断と治療が必要である．

　Patients with IBD（inflammatory bowel disease）who are treated with immunosuppressive medications such as corticosteroid and infliximab have concomitant infectious risks. IBD patients with enteric infections have many problems because a complex pathology makes diagnosis difficult, and leads to relapse, severe and refractory disease. Enteric infections complicating IBD include cytomegalovirus, norovirus, Clostridium difficile, Salmonella spp., Campylobacter spp., Entamoeba histolitica, Giardia lamblia, Strongyloides stercoralis, Cryptococcus. We should suspect enteric infections from clinical and endoscopic findings, and make appropriate diagnosis and treatment of the diseases.
