Macroscopic Classification of Early Colorectal Cancer

要旨　大腸でも陥凹型病変（IIc）が存在することが注目された折，報告者間でこのような病変に対する認識が一定しておらず，少なからず混乱を生じた．大腸癌に関する組織発生，発育と増殖，診断と治療などに関する討論が行われる場合，共通言語がなければ誤解を生じることが痛感された．そこで，大腸癌研究会では大腸癌，特に表在型腫瘍の肉眼型分類などの定義を作成するために，「表在型大腸腫瘍プロジェクト研究班」を結成して討論した．そのコンセンサスは「大腸癌取扱い規約第7版」に記載された．わが国の研究成果を世界に発信するに当たって，大腸癌研究会における肉眼型分類作成の理念を踏襲して，共通言語を用いた論文作成がなされることを期待する．

　Since 1977, the Japanese classification of colorectal carcinoma has been reported by the Japanese Society for Cancer of the Colon and Rectum（JSCCR）. In this classification, general rules for clinical and pathological studies on cancer of the colon, rectum and anus have been decided. Macroscopic types of early colorectal cancer were described as type 0. During the last 20 years, many depressed early cancers have been reported in Japan. To define depressed type colorectal cancer, a working group in the JSCCR has been established and has discussed the re-definition of the macroscopic classification of early colorectal cancer. The details of the new criteria are described in the 7th edition of the JSCCR. In discussing the diagnosis, treatment and pathology of early colorectal cancer, we should use these new criteria.
