 With the increasing number of submucosal(sm)invasive colorectal cancer cases by endoscopic dissection, it is critical to determine the criteria for curative resection. Although vascular infiltration is considered the most reliable predictive factor of lymph node metastasis, determination of vascular infiltration may vary among different pathologists. We have established strict criteria for lymphatic and venous infiltration. Multivariate analysis in 480 cases of submucosal invasive colorectal cancer in which these criteria were applied indicated the strongest correlation between lymphatic infiltration and lymph node metastasis. However, venous infiltration was not an independent predictive factor of lymph node metastasis. With the development of instruments to distinguish lymphatic vessels from veins for vasculature with small diameter, the implications of each lymphatic and venous infiltration should be evaluated carefully.
