 “Tumor budding/sprouting”is a strong predictive factor for lymph node metastasis of submucosal invasive colorectal cancer. In the study for interobserver reproductivity, tumor budding/sprouting revealed a higher kappa value than lymphatic permeation. In some cases, it is difficult to differentiate cancer cells from fibroblast or endothelial cells in desmoplastic reaction, and macrophages in inflammation.

 Immunohistochemistry using anti-cytokeratin antibody is very useful for identifying such difficult cancer cells. To make tumor budding/sprouting a more significant factor, however, we should clarify the usefulness of immunostaining and adequate cutoff point by analysis of a large number of cases in the future.
