大腸SM癌の病理診断とその問題点―断端の評価

Pathological Diagnostic Issues Concerning the Vertical Margin of Submucosal Cancer in the Colon and Rectum

要旨　内視鏡下切除を行った結果，癌がSM断端近傍まで認められた，または断端に露出していた場合，その後の追加腸切除が必要であるかを知るために，内視鏡下切除を行い，その後に外科的追加腸切除が行われた大腸SM癌45病変を対象として，SM断端の状態とリンパ節転移や追加腸切除時の腸管内癌遺残などの関係について検討した．その結果，45病変中5病変（11.1％）にリンパ節転移を認め，そのうち4病変がSM断端500μm未満または断端陽性であった．またEMR後追加腸切除された症例で，検討が可能であった35病変中，SM断端500μm未満の1病変およびSM断端陽性の1病変において腸管内に癌の遺残が認められた．以上のことからSM断端500μm未満およびSM断端陽性の病変では外科的追加腸切除を行うべきであると考えられた．

　In the case of positive vertical margin of submucosal cancer in EMR, is there necessity for radical operation? We evaluated the relationship of vertical margin and lymph node metastasis, residue of cancer in the colon and rectum. In our investigation, we focused on 45 submucosal colorectal cancers. Lymph node metastasis was found in 5 cases（11.1％）, 4 of which were positive or almost positive for vertical margin. Residue of cancer in the resected colon and rectum after EMR was found in 2cases（5.7％）, one of which was positive and another was almost positive for vertical margin. So, in cases of positive or almost positive vertical margin of submucosal cancer in EMR, radical operation is necessary.
