要旨　三角ナイフは，ESDの先端処置系の道具であり，3方向に尖っているため，軸合わせの必要のないナイフである．基本手技は，標的の組織を三角プレートの角にフック様に引っ掛けて，消化管内腔側に筋層表面から挙上して，電気メスで切離する．マーキング，プレカッティング，周辺切開，粘膜下層剝離などの一連の手技をこれ1本で行う．マーキングのみはプレートを外筒に格納したままで，Forced coagulation 40W，effect 3（ICC200，VIO，ERBE）で行い，その後の一連の手技（プレカッティング，周辺切開，粘膜下層剝離など）はSwift coagulation 40W，effect 4（VIO，ERBE）で行う．ESDに際して，使用する処置具は，局注針，三角ナイフ，止血鉗子の3つがあれば実施可能である．本稿では，三角ナイフを用いたESDの実際と成績を概説する．

　The “triangle-tip” electrocautery knife has been newly designed as a tool which can pass through the working channel of the endoscope and used for endoscopic submucosal dissection (ESD) of upper gastrointestinal intramucosal neoplasia. All processes of ESD including marking, marginal cutting, submucosal dissection and hemostasis can be performed by this single device. 199 cases of gastric cancer and 102 cases of esophageal tumor received ESD using a triangle-tip knife. In all cases ESD was successfully performed with no serious complications. We consider that this novel device is another useful endoscopic instrument for ESD. Comparative study with other devices will be a useful and necessary measure for future evaluation of ESD devices.
