食道ESDの実際―三角ナイフ

Endoscopic Submucosal Dissection Using the Triangle Tip Knife

要旨　三角ナイフは，先端に三角形の刃を配置したいわゆる先端系ナイフであり，① マーキングから切開・剥離までの一連の操作を単一のデバイスで行う，② 器具の使用に際して軸合わせが不要，の2点を意図して開発された（Fig. 1a, b）．基本手技は，標的の組織をフックし，管腔側に挙上し，通電する，の反復であり，三角ナイフ1本のみ処置が可能である．当センターでは，2001年4月の開院以来2008年8月31日までに，食道のEMR 168件，ESD 89件，計257件を施行し，ESDのうち穿孔は1件（1.1％），後出血は0件（0％）である．穿孔率・出血率とも低い数字を保っており，三角ナイフは合併症の少ない安全な処置具であると考えている．また，一括切除，再発においても有用であると考えられる．

　The triangle tip knife has a triangle-shaped blade on the tip of it. The knife can achieve almost the entire process of ESD including marking, circumferential cutting and resection. This simple knife doesn't need to have its direction fixed. The procedure consist of only three simple moves “hook the tissue, lift it up and electrify to cut it."

　We have experienced 168 cases of esophageal EMR and 89cases of ESD since Apr. 2001 and only one（1.1%）perforation has been experienced. No recurrence in ESD cases have been reported.

　The triangle tip knife contributes to easier and safer en bloc ESD.
