副咽頭間隙に発生したきわめてまれな唾液腺導管囊胞の1例

An extremely rare case of salivary duct cyst developed in the parapharyngeal space

はじめに

　副咽頭間隙腫瘍は全頭頸部腫瘍の0.5％とまれな疾患である^1,2）。組織型は多彩であることが知られており，唾液腺由来の腫瘍と神経原性腫瘍が多い^3〜5）。ドイツからの報告では，唾液腺疾患のうち囊胞性疾患は6〜9％とまれであり，唾液腺導管囊胞はさらにそのうちの9.6％であった^6）。つまりドイツでは導管囊胞は唾液腺疾患のうちの0.576〜0.864％であった。本邦でもその頻度は0.5％と報告されている通り^7），副咽頭間隙に発生した唾液腺導管囊胞は非常にまれと思われ，われわれが得た検査所見や選択した手術方法を報告する。

　A 59-year-old man who experienced recurrent episodes of swelling of the left parotid region that started 6 months before presentation visited our facility. The examination revealed a lesion extending from the deep lobe of the parotid gland to the parapharyngeal space. The patient underwent surgical excision with an external incision. The histological diagnosis was salivary duct cyst. The postoperative course has been favorable without obvious complications or recurrence. When selecting treatment options for a parapharyngeal space tumor, the possibility of a rare disease, such as the one in this study, needs to be kept in mind.
