好酸球性副鼻腔炎に合併した頭蓋底に付着する篩骨洞骨腫の1例

A case of paranasal sinus osteoma in eosinophilic sinusitis

はじめに

　副鼻腔に発生する骨腫は多くが無症状であり，潜在的に骨腫を有していることも少なくない。治療は手術による摘出が必要であるが，その手術法に関する報告は比較的少ない。今回われわれは，好酸球性副鼻腔炎を伴った頭蓋底に付着する骨腫症例を経験し，内視鏡下鼻内手術で摘出できたため，若干の文献的考察をふまえて報告する。

　A 78-year-old woman underwent endoscopic sinus surgery for eosinophilic sinusitis with cribriform osteoma.

　During surgery, bony seeds were removed to improve ventilation. Extraction revealed a bone wall defect at the skull base. The dura mater was exposed, but no reconstruction was performed because no cerebrospinal fluid leakage was observed. The patient had a good postoperative course and no complications were observed. The decision to remove a sinus osteoma should be made after carefully considering the necessity, invasiveness, and management of complications associated with the removal.
