アルコール性小脳失調症の断酒後長期経過観察例

A case of the follow-up long-term after abstinence of alcoholic cerebellar degeneration

はじめに

　アルコール性小脳失調症は，長期間の飲酒により生じる小脳失調症である。過去にはアルコール依存症・中毒症症例における平衡機能の統計学的報告が存在し，電気眼振図検査（electronystagmography：ENG）においてeye tracking test（ETT），optokinetic nystagmus test（OKN）などでの小脳障害の異常出現率が半数以上であったこと，断酒後3年以内の症例では異常出現率が初回とほとんど変わらないか，やや減少していることが報告されている^1,2）。しかし，断酒後3年以上の長期経過については明らかとなっていない。

　そこで今回われわれは，29歳時にアルコール性小脳失調症と診断され，それ以降断酒し，現在まで38年間経過観察を行っている症例について報告する。

　A patient was hospitalized for gait disturbance when he was 29 years old, and alcoholic cerebellar degeneration was diagnosed. Thereafter, he stopped consuming alcohol. Subsequently, balance tests have been performed regularly until this year, when he turns 67 years. To date, the cerebellar degeneration findings have not shown progression on electronystagmogram examinations. This is a valuable case report demonstrating that quitting alcohol consumption is very important for patients with alcoholic cerebellar degeneration.
