喉頭浮腫で受診し好酸球増多症候群と診断された1例

A case of laryngeal edema diagnosed with a hypereosinophilic syndrome

はじめに

　好酸球増多症候群（hypereosinophilic syndrome：HES）とは，1968年にHardyとAndersonにより概念が提唱された，好酸球増多に加え，組織中の好酸球増多により臓器障害を呈し，かつ臓器障害をきたすほかの原因がない疾患のことである^1〜3）（表1）。皮膚，肺，消化管，心臓，神経などが主な標的となる^2）が，われわれが渉猟しえた限りでは喉頭浮腫を呈した報告は認めなかった。HESの治療は一般的に副腎皮質ステロイドが第一選択となることが多く，副腎皮質ステロイドに抵抗性の場合は免疫抑制薬やIFN-α，抗IL-5抗体などを使用する。またFIP1L1-PDGFRA融合遺伝子異常がある場合はイマチニブが奏効するといわれている^4,5）。

　今回われわれは，HESが原因と考える喉頭浮腫を認めた症例を経験したため，若干の文献的考察をふまえて報告する。

　Hypereosinophilic syndrome（HES）is a disease characterized by hypereosinophilia with no other cause of organ damage.

　We report the case of a 62-year-old female patient with laryngeal edema who was diagnosed with HES by histological diagnosis after tracheostomy.

　The eosinophil count had been elevated since the time of admission, tissue invasion of eosinophils were observed at the biopsy site, and we started treatment with PSL（0.5mg/kg）The laryngeal edema gradually improved, tracheostomy was closed, and the patient was discharged from the hospital.
