Abstract Purpose:To report a case of linezolid(LZD)-induced optic neuropathy associated with prolonged use of LZD for multidrug-resistant pulmonary tuberculosis.

Case:Optic neuropathy occurred in a 61-year-old woman who was undergoing prolonged polypharmacy, including LZD for multidrug-resistant pulmonary tuberculosis. The best corrected visual acuity(BCVA)was 0.04 in both eyes. The critical fusion frequency(CFF)was 13 Hz in the right eye(RE)and 14 Hz in the left eye(LE). No obvious abnormal findings were found on the anterior segment, fundus, and head magnetic resonance imaging. LZD-induced optic neuropathy was suspected, and the polypharmacy was discontinued. Two months after discontinuation, the BCVA was improved to 1.0 in the RE and 1.2 in the LE. Following the improvements in the patient's vision, polypharmacy for tuberculosis resumed, excluding LZD. The BCVA was 1.2 in the RE and 1.2 in the LE, the CFF was 30 Hz in the RE and 27 Hz in the LE 11 months after the discontinuation of LZD.

Conclusion:LZD can cause drug-induced optic neuropathy, which should be kept in mind, particularly for cases requiring prolonged treatment.
