要旨

膵癌は最も予後の悪い癌の一つと考えられている。早期診断が難しく，容易に進展転移するため，およそ半数が診断時Stage Ⅳである。一方，Stage Ⅰは1割にも満たず，早期診断は非常に困難である。また，T1a膵癌でも高率にリンパ節転移や遠隔転移を認めている。手術治療単独での長期生存率は決して高くない。現在は術前術後補助化学療法に手術治療を組み合わせた集学的治療が中心になり，良好な成績を収めている。本邦ではS-1による術後補助化学療法とGS療法による術前補助化学療法が標準である。一方，根治切除ができない膵臓癌に対しては化学療法が選択される。近年，有効なレジメンの出現に伴い，化学療法の成績が向上し切除可能となる患者が増えてきている。Conversion surgeryにより，今まで長期生存が見込めなかった患者も，長期生存できる可能性が高まり期待されている。膵癌の早期発見を目指して，地域医療連携を生かした報告が散見される。地域連携施設で膵癌の危険因子を複数有する患者に対して精査を行う。この取り組みにより，膵癌の早期発見率が向上し，生存率が改善している。

　Pancreatic cancer is one of the cancers with the worst prognosis because of the difficulty of early diagnosis and the ease of progression and metastasis. Early diagnosis of Stage I pancreatic cancer is extremely difficult. Furthermore, lymph node metastases or distant metastases frequently occur, even if it is a case of T1a pancreatic cancer. The long-term survival rate after surgical treatment alone is not good. A multidisciplinary treatment combining neoadjuvant chemotherapy using GS, postoperative adjuvant chemotherapy using S-1, and surgery has achieved good results.

　Chemotherapy is the first-choice treatment for unresectable pancreatic cancer. Conversion surgery is expected to increase the possibility of long-term survival.

　Recently, several reports have discussed the effectiveness of utilizing regional medical cooperation for the early detection of pancreatic cancer. Patients with multiple risk factors for pancreatic cancer are examined closely at regional cooperative facilities. This initiative has increased the early detection rate of pancreatic cancer and improved survival rates.
