Surgical Approach for Non-ampullary Duodenal Adenoma and Carcinoma

要旨●リンパ節転移を考慮せずともよい十二指腸腺腫や粘膜内癌では，局所切除（内視鏡的切除あるいは外科的切除）で治療を完結できる．十二指腸腫瘍に対する外科的切除法には，根治術とされる膵頭十二指腸切除術の他に各種縮小手術（経十二指腸的粘膜切除術，全層部分切除術，十二指腸切除術など）がある．また，多くの縮小手術では腹腔鏡下アプローチも可能である．近年では，内視鏡的切除と腹腔鏡下での漿膜側補強縫合を付加する術式も開発され，新しい縮小手術として期待されている．内視鏡的切除が困難，あるいは危険と判断される腺腫や粘膜内癌は，多くの場合でこれらの縮小手術で対応可能であり，膵頭十二指腸切除術が必要な局面は多くない．

　Surgical resection is the treatment of choice for non-ampullary duodenal adenoma and carcinoma that is not amenable to endoscopic resection due to technical and/or oncological reasons. Duodenal adenoma, as well as intramucosal carcinoma, rarely metastasizes to the lymph nodes ; therefore, even when surgery is indicated, minimal local resection that is as small as possible without lymph node dissection, such as transduodenal mucosal excision, local full-thickness resection, pancreas-sparing segmental duodenectomy, or reinforcement of seromuscular/full layer after endoscopic resection, should be performed as an alternative to pancreatoduodenectomy. A laparoscopic approach can be applicable to almost all limited surgeries.
