専攻医の耳学問の場― “よろず相談室” の導入とその教育効果―

The Implementation of the Yorozu Consultation Room and Its Educational Impact

はじめに

　近年，働き方改革の推進により，専攻医1人あたりが経験できる症例数や手技の機会は制限されつつある。その結果，限られた臨床経験の中で，いかに効率的かつ質の高い教育を提供するかが，教育現場における重要な課題となっている。

　かつて，多くの麻酔科医局では，日常診療の合間に先輩麻酔科医との会話を通じて知識や判断の勘所を学ぶ，いわゆる “耳学問” が自然発生的に行われていた。そこでは，教科書や論文には明示されにくい臨床判断の背景や工夫といった暗黙知が共有され，日常診療に直結する学びが継承されてきた。しかし近年，勤務環境や時間的制約の変化により，このような非公式な学習機会は減少している。

　一方，効率的な学習を志向する若手世代が増えるなかで，ベテラン世代は，一見非効率に思われがちな対話や雑談の中にこそ，臨床に直結する重要な学びが含まれていることを経験的に理解している。

　名古屋市立大学麻酔科では，こうした背景を踏まえ，失われつつある “耳学問” の価値を再評価し，その学習機会を組織的に再構築することを試みた。その取り組みが，専攻医を中心とした教育の場である “よろず相談室” である。

　Work-style reforms have reduced clinical exposure for anesthesiology residents, highlighting the need for efficient educational strategies. Informal “learning by listening,” once common in anesthesiology departments, facilitated the transfer of tacit clinical knowledge but has become less frequent. In 2024, we introduced an educational forum, the ‘Yorozu Consultation Room,’ at Nagoya City University. The forum’s weekly 30-minute sessions involve anesthesiology residents and attending physicians and include resident-led case presentations, discussions, and senior physicians’ feedback. The program was designed as a shared experiential learning platform. Past sessions have addressed daily clinical questions, perioperative complications, and examination-style case discussions. A survey of eight residents demonstrated positive educational effects, including improved knowledge organization（87.5％）and learning from peers’ experiences（87.5％）. The Yorozu Consultation Room is a concise and practical experiential learning model that promotes reflection, shared learning, and psychological safety within a limited training time period.
