救急科専門医，集中治療科専門医を目指す若手麻酔科医へのメッセージ

A Message to Young Anesthesiologists Considering a Career in Emergency and Critical Care Medicine

はじめに

　麻酔科医の周術期・急性期医療への知識や経験が生かされる領域が拡大しており，手術麻酔に割く労力を技術革新やチーム医療で軽減することで，現状の手術麻酔の医療の質を維持したうえで，麻酔科医に求められる手術室外での需要に応え，活躍の場を広げていく必要がある^1）2）。

　若手麻酔科医にとって，麻酔科専門医の資格取得後にキャリアの選択肢を広げることは重要である。特に，重症患者管理に関心をもつ麻酔科医にとって，“救急科専門医” や “集中治療科専門医” の資格取得は有力なキャリアパスの一つとなる。

　救急医療や集中治療の現場では，重篤な患者に対する迅速な対応が常に求められている。麻酔科医のもつ気道管理，疼痛管理などの技術に加えて，手術麻酔という恒常性の破綻した状態を常にモニタリングしていることによる全身状態の把握，全身状態の恒常性の維持の知識は，救急・集中治療との互換性が高く，これらの分野で非常に有効である。また，現代の救急・集中治療においては，ほかの医療専門職との連携が不可欠であり，普段から主治医や看護師，薬剤師をはじめとする多職種連携を行い，手術室における全身管理のイニシアチブを担う麻酔科医の強みも，この領域で発揮することが期待されている。

　本論文では，“救急科専門医” および “集中治療科専門医” を目指す若手麻酔科医に向けて，サブスペシャルティ取得への具体的な道筋，必要なスキルについて述べる。

　Anesthesiologists’expertise in perioperative and acute care has expanded, highlighting the need to optimize their efforts through advancements in technology and collaborative team-based approaches. For anesthesiologists interested in emergency and critical care medicine, an expansion of their roles outside the operating room is becoming increasingly important. For young anesthesiologists, obtaining certifications in emergency or intensive care medicine opens up promising career paths.

　In addition to their skills in airway management, pain control, and more, anesthesiologists have extensive knowledge in monitoring patients in states of disrupted homeostasis during surgery and anesthesia, as well as in maintaining homeostasis overall. These skills and knowledge are highly compatible with the fields of emergency and critical care medicine, where they prove to be exceptionally valuable. I strongly encourage more anesthesiologists to consider advancing into the areas of emergency and critical care medicine in the future.
