はじめに

　ここ10年余りで多くの施設において最小侵襲手術（minimum invasive surgery）が可能になった。内視鏡手術は創部を小さくすることができ，早期治癒につながる。しかし，小児にとっては低侵襲とは言いがたい。視野を改善するために行われる人工気腹や人工気胸は，さまざまな臓器に物理的な圧力を与える。この圧力は空間にかかる絶対圧に加え，時間の因子にも依存する。また，これらの手術は臓器だけでなく，血管系にも影響を及ぼす。そこで，生体に及ぼす影響や関連する因子，そして麻酔管理法について説明したい。

　Minimally invasive surgeries have recently become more common, but it must be noted that surgery for smaller wounds is not as minimally invasive for children as it is for adults. Artificial pneumoperitoneum and pneumothorax, which are performed to improve surgical conditions, apply physical pressure to various organs；this pressure also influences the vascular system. I would like to discuss the effects of artificial pneumoperitoneum and pneumothorax on the organs involved, influencing factors, and anesthesia management in pediatric patients undergoing endoscopic surgery.
