Treatment and Management of Chronic Pain Patients：Preface and Comments

　「慢性疼痛患者への治療と対処」と題する特集を取り上げたのは2008年のことで15年前のことになる。その間，さらなる社会の高齢化とともに，当然のことながら慢性疼痛患者数の増加がみられている。その数およそ2,000万人といわれているが，その経済的損失も年間4兆円に上ると考えられている。そのために慢性疼痛疾患への対策が重要な課題となり，診断の場において，体温，血圧，脈拍，呼吸の基本的バイタルサインに加えて，第5のバイタルサインとの考えもあり，実臨床の場においても疼痛に強く関心がもたれるようになってきた。しかも近年，新しく慢性疼痛に対する適用が認められた強オピオイド鎮痛薬や新たなトラマドール製剤も使用されるようになってきたこともあり，ますます慢性疼痛への対策の必要性の増加がみられてきている。

　Chronic pain is well known to be very difficult in management；because the mechanisms of chronic pain are so much complicated. Therefore, chronic pain patients are often involved with multiple pain mechanisms and need multiple drugs which have different types of analgesic actions, besides, rehabilitation, interventional treatment, light therapy and so on. The evaluation of many factors which modulate the pain behavior is very important.
