  • Abstract

 Japanese society today is experiencing an unprecedented demographic composition including an extremely high proportion of elderly people. Under these circumstances, the number of elderly people undergoing surgical operations is increasing. Therefore, we need to recognize age-related physiological changes in each organ. The pharmacokinetics and pharmacodynamics of intravenous anesthetics in elderly patients are also crucial for the adequate management of clinical anesthesia.

 From that perspective, it is necessary to evaluate whether an elderly patient is frail or not before surgery in order to select an appropriate surgical procedure. The number of elderly patients suffering from chronic pain has increased. It is often difficult to identify the cause of their pain in routine medical care, and they sometimes resist various pain treatments. Effective pain management in the elderly requires a tailored rehabilitation approach attuned to the distinctive physiological and neurological transformations accompanying the aging process. In emergency medicine, it can be difficult to transfer elderly patients to other hospitals or facilities, and in recent years social problems have become challenging. In palliative care, end-of-life decision-making and advance care planning(ACP)are becoming increasingly important. ACP is a process in which patients identify their goals and wishes for future treatment and care based on their own values and discuss these with their families and healthcare professionals. Finally, health management issues and status for elderly people are described, and the difficulties of promoting ACP are considered from a socio-medical perspective.
