横紋筋肉腫は小児に発生する軟部肉腫の中で最も頻度が高く，頭頸部は好発部位の1つである^1）2）。頭頸部発生の横紋筋肉腫は時に頭蓋内進展を呈し，さらに脳神経浸潤が認められることが報告されている^3）。今回我々は，錐体骨に発生し，頭蓋内進展および脳神経浸潤を呈した横紋筋肉腫の1例を経験したので，文献的考察を加えて報告する。

We report a 12-year-old boy with embryonal rhabdomyosarcoma of the petrous bone, presenting with left trigeminal and abducens nerve palsies. CT showed an ill-defined osteolytic mass involving the petrous bone and clivus, with enlargement of the foramen ovale. MRI revealed heterogeneous enhancement, extending from the petrous apex through Meckel’s cave to the foramen ovale, indicating cranial nerve involvement, and spreading from the clivus into the extracranial/parapharyngeal space. This case highlights cranial nerve involvement as a key imaging feature in head and neck rhabdomyosarcoma.
