頸部悪性リンパ腫による頸動脈洞症候群に対して早期放射線治療が有効であった1例

A case of prompt radiotherapy for carotid sinus syndrome associated with cervical malignant lymphoma

頸動脈洞症候群（carotid sinus syndrome：CSS）は，頸動脈洞の圧迫により圧受容体が刺激され，迷走神経反射が過剰に生じることで循環障害を引き起こす病態である^1）。主な臨床症状は血圧低下や神経調節性失神であり，失神を伴う場合には致死的転帰をとることもある^2）。失神の原因疾患としてCSSが占める割合は全体の約5.8％にすぎず^3），見逃されやすい病態であるが，失神の鑑別診断において重要である。

We report a case of diffuse large B-cell lymphoma（DLBCL）of the neck presenting with carotid sinus syndrome（CSS）, successfully managed with early radiotherapy. Radiotherapy was promptly initiated after pathological diagnosis to address syncope and related symptoms, enabling a smooth transition to systemic chemotherapy. The patient achieved complete remission of DLBCL, with no recurrence of CSS during follow-up. This case highlights the importance of rapid, adaptable treatment strategies for head and neck tumors presenting with syncope, and suggests that early radiotherapy may serve as an effective initial intervention in such oncologic emergencies.
