IVRとは，欧米ではIRと略されるinterventional radiologyのことで，X線，超音波，CTといった画像診断法を用いてほぼ全身の臓器を治療する，専門領域である^1）。

This article outlines the evolution and current practice of pediatric interventional radiology（PIR）, with a focus on abdominopelvic interventions. It introduces the global development of PIR, particularly in the United States, and compares it to Japan’s current state, where PIR remains integrated within diagnostic radiology. Examples from the author’s institution are presented, covering vascular and non-vascular PIR procedures such as post-transplant interventions, congenital extrahepatic portosystemic shunt, biliary drainage, and pelvic abscess management. At the author’s institution, PIR procedures are typically performed in close collaboration with attending clinicians. The article emphasizes the importance of dedicated pediatric radiologists with IR expertise in delivering safe, effective PIR.
