Representative interventional radiology(IVR)techniques for lung cancer include radiofrequency ablation(RFA), microwave ablation(MWA), and cryoablation. As of 2025, RFA for lung cancer is covered by health insurance in Japan. However, in patients with coexisting interstitial pneumonia, there is a risk of acute exacerbation of interstitial pneumonia after RFA. According to the results of a recently reported multicenter retrospective study conducted in Japan, the incidence of acute exacerbation of interstitial pneumonia following RFA was 8%, and the mortality rate among those who developed it was 60%. Therefore, careful consideration is required when determining the indication of lung RFA for patients with interstitial pneumonia. In the future, evidence is expected to be established regarding risk factors for the development of interstitial pneumonia that can be assessed before RFA, as well as treatment strategies to reduce the risk.
