後縦隔ミュラー管嚢胞は2005年にHattoriが報告した縦隔嚢胞性疾患の比較的新しい概念である^1）2）。後縦隔発生の嚢胞性疾患としてミュラー管嚢胞の他に，気管支原性嚢胞や変性した神経原性腫瘍，食道嚢胞などが挙げられる。特にミュラー管嚢胞と同じ線毛上皮を有する気管支原性嚢胞との鑑別が問題となるが，エストロゲンレセプター（ER）とプロゲステロンレセプター（PgR）による免疫染色が鑑別に有用である。今回我々は，後縦隔ミュラー管嚢胞の2例を経験したので報告する。

A mediastinal Müllerian cyst was initially reported as a new category of congenital cyst by Hattori. in 2005. These cysts are frequently mistaken with other thoracic cysts such as bronchogenic cysts, degenerating neurogenic tumors, and esophageal cysts because of the poor clinical picture and nonspecific radiologic presentation. The cysts are confirmed to be of Müllerian origin by ER and PgR staining. It is important for the radiologist to list Müllerian duct cysts in the differential for diagnosis. Here, we report two cases of incidental findings of mediastinal Müllerian cysts.
