2002年にFDG-PET検査が特定の悪性腫瘍，虚血性心疾患，てんかんの診断を対象として保険適用とされて以降，徐々にその適応は拡大している。FDGが炎症組織に集積することは広く知られていたが，悪性腫瘍の良悪診断が保険適用に至らなかったのは，腫瘍のみならず炎症にもFDG集積が認められることにより，その鑑別が困難と判断されたためである。2012年にFDG-PETは心サルコイドーシスの炎症部位診断に対して保険適用となり，炎症の診断に門戸が広がった。2018年には「大型血管炎と診断された患者で，病変の局在または活動性の診断（可視化）」を目的としてFDGは保険適用となり，いわゆる炎症性疾患に対してPETの適応が拡大された。現在，我が国ではFDGによる不明熱の診断などについて保険適用を目指す活動があるが，欧州主要国における炎症に対するFDGの適用に鑑みれば，それは公知の事柄と解釈され，我が国でもいち早い保険適用が期待される。本稿では，FDG-PET保険収載20周年と今後の展開を考えるうえで，炎症PETの推進に関して論じる。

Since the FDG-PET examination was covered by health insurance for the diagnosis of several types of malignant tumors, ischemic heart disease, and epilepsy in 2002. In 2012, FDG-PET became covered by insurance for the identification of the inflammation site of cardiac sarcoidosis, indicated that FDG was opened the door to the diagnosis of inflammation. In 2018, FDG was covered by insurance for the purpose of diagnosing the localization or activity of lesions in patients diagnosed with large vessel vasculitis, and the application of PET was expanded for so-called inflammatory diseases. Currently, there are promotion aimed at applying insurance for the diagnosis of fever of unknown origin by FDG, but it is widely known worldwide that it has been approved in major European countries.
