副脾は捻転により梗塞を起こすことが報告されている。臨床症状としては捻転および自然解除による間歇的な腹痛が特徴である。特異的な画像所見は乏しく，通常その他の腫瘍との鑑別は困難であると考えられるが，脾門部から連続する栄養血管の存在から術前診断し得た報告もあり，局在や経過から本病態の可能性を考慮して診断を行うことが重要である。副脾の梗塞はまれな病態であり若干の文献的考察を加えて報告する。

A 26-year-old man was admitted to our hospital due to abdominal pain. Contrast-enhanced CT demonstrated a non-contrast-enhanced mass in the splenic hilum. One month later, the mass became smaller, and the margin of the mass was enhanced in the contrast-enhanced CT. A gastric gastrointestinal stromal tumor was suspected, and laparoscopic surgery was performed. Pathologically, splenic tissue was found in the mass. The final diagnosis was infarction of an accessory spleen.
