腹痛にて発症し大網腫瘍と鑑別困難であった副脾茎捻転の1例

A case of torsion of an accessory spleen mimicking omental tumor with abdominal pain

副脾は脾臓の先天性形態異常の一つで，剖検例の10～30％^1）に存在すると報告されている。日常の腹部CTでも円形構造として脾臓周囲に遭遇することが多い。ほとんど臨床的に問題となることはないが，骨盤腔など非典型的な位置に発生した場合，他の腫瘤との鑑別が問題となり，術前に診断されることはまれである。また，ときに捻転をきたし，自覚症状を示すこともある。

A 57-year-old man was admitted due to left side abdominal pain. Abdominal ultrasonography, contrast-enhanced CT, MRI demonstrated an enhanced solid mass with hemorrhagic necrosis in the grater omentum, and it showed low FDG accumulation in PET. It was difficult to differentiate from omental tumor. Laparotomy was performed and showed a mass with a long twisted pedicle. Pathologically, it contained splenic tissue with degeneration. The final diagnosis was torsion of an accessory spleen with infarction.
