定位放射線治療は多方向から少量の放射線を高精度に集中させて周囲の正常組織への線量を低く抑えて病巣に十分な放射線量を投入するという治療法である。定位放射線治療は頭蓋内病変に対して開発されたいわゆる「ガンマナイフ」がはじまりで，1968年から臨床応用されるようになった^1）。しかし，呼吸性の移動があり，固定の難しい体幹部病変に応用されるようになったのは1990年代に入ってからである^2）3）。精度の高い放射線治療により，リスク臓器への線量の低減を図り，高線量を病巣に照射することが可能となり，臓器によっては明らかな制御率の改善が得られるようになった^4）。体幹部定位放射線治療（stereotactic body radiotherapy：SBRT）は2004年4月に我が国で保険収載されてから，大規模な病院だけでなく小規模な施設においても広く一般に行われるようになった。2020年4月には保険適用が拡大され，SBRTはさらに多く病態に適応できるようになってきた。

Stereotactic body radiation therapy（SBRT）is now a frequently used modality for the treatment of early stage non-small cell lung cancer and oligometastatic disease of the lung. The radiological changes of the lung after SBRT are likely to differ from those observed after the conventional thoracic radiotherapy. It is mainly because of the smallness of target and highly conformal dose distributions with steep dose gradients from the target to surrounding normal lung tissues. Knowing the radiological changes after SBRT is needed in order to correctly diagnose local failure. Several specific radiological changes after SBRT that should be known were reported in this paper.
