放射線肺障害は1920年頃にはすでに報告され，胸部の放射線治療後に肺炎症状を呈し，肺線維化を残すという経過が知られていた。近年放射線治療患者数が増加し，胸部放射線治療による合併症として放射線肺障害に接する機会が増えてきている。本稿では，放射線肺障害についてその背景と典型的な画像について解説する。

It is well described about radiation induced pulmonary injury that complicates after thoracic radiation therapy. It is mainly a series of phenomena caused by alveolar injury by irradiation. Clinical findings include only spontaneous ground glass appearance without any symptoms as well as a rush and fatal course. In this article, we present three cases on asymptomatic radiation pulmonary fibrosis, radiation related BOOP, and radiation pneumonitis, including commentary on the mechanism of radiation pneumonitis.
