当院では2017年より放射線治療装置用動体追跡システムSyncTraX FX4^®（島津製作所社製）を導入し，待ち伏せ定位放射線治療を開始した。導入当初，設定した位置にマーカが移動せず長い待機時間を要した症例を複数経験したため，その要因を行程ごとに検討した。要因として，高齢者，患者固定具の精度不良，患者の筋緊張による呼吸変動，計画CT時の呼気停止画像と照射時自由呼吸の呼気画像との相違が挙げられ，各事項に対し対策を検討した。

We started the SBRT（stereotactic body radiotherapy）using SyncTraX FX4（real-time marker tracking system）at July 2017. We found that the marker position at planning did not conform to that at treatment, and SBRT required longer than one hour. The causes consisted of high age, insufficient immobilization, the difference between exhalation phase at planning and that at treatment.
