関節リウマチを背景にした多発nodular lymphoid hyperplasiaの1例

nodular lymphoid hyperplasia（NLH）は肺のリンパ組織であるbronchus-associated lymphoid tissue（BALT）を発生母地として，炎症などの外的刺激による影響で生じるまれな良性リンパ増殖性疾患と考えられている。悪性リンパ腫や原発性肺癌との鑑別がときに難しく，外科的切除にて診断がなされることもある。まれな病変であるため，そのまとまった画像所見の特徴や報告は少なく，その自然経過についてあまり知られていないのが現状である。今回，関節リウマチ（rheumatoid arthritis：RA）を背景とした多発NLHの自然経過を追うことのできた1例を経験したため，文献的考察を加え報告する。

Nodular lymphoid hyperplasia（NLH）is thought to be rare benign lymphoproliferative lesions that originates from bronchus-associated lymphoid tissue（BALT）by external stimuli such as inflammation. Because its CT findings are sometimes very similar to that of adenocarcinoma, surgical resection is often performed. Since NLH is very rare, the natural course of the disease is still unknown. We report a case of multiple NLH which was followed up for several years and showed characteristic morphological changes. Our findings may contribute to implications for understanding the natural history of NLH.
