肺動脈本幹部に腫瘤を認めた際には，肺血栓塞栓症と肺動脈腫瘍を鑑別することは治療方針決定のために重要である。両者の鑑別には¹⁸F-FDG-PET/CTが有用であるとの報告がある^1-3）。今回，FDG高集積のみられた肺動脈本幹部の真菌血栓症の1例を経験したので報告する。

We report a case of pulmonary thromboembolism with fungal infection, differential diagnosis from the malignant tumor of the pulmonary artery was difficult due to the FDG uptake on PET/CT. The patient is a man in his 60s with myelodysplastic syndromes. Contrast-enhanced CT at admission showed mass formation in right pulmonary artery. Enhancement of the lesion was not observed by contrast-enhanced CT and MRI. ¹⁸F-FDG-PET/CT showed high uptake of FDG to the lesion, mimicking malignancy. To make a definitive diagnosis, endovascular catheter biopsy was performed. The diagnosis was pulmonary thromboembolism with fungal infection.
