片麻痺性片頭痛は運動麻痺（脱力）を含む前兆のある片頭痛と定義され，小児において急性の局所神経症状を呈する原因疾患の一つではあるが，画像診断は除外診断目的で用いられることが多い。その病態生理は複雑で完全には解明されていない。今回我々はASLが診断に有用で，画像で経過を追うことができた片麻痺性片頭痛の1例を経験したので報告する。

The patient was 14-year-old boy with headache and focal neurologic symptoms. MRI on the attack demonstrated hypoperfusion in the right hemisphere on ASL. Follow-up MRI revealed the phenomenon of rebound hyperperfusion. Many studies have shown initial hypoperfusion in acute phase, followed by delayed hyperperfusion in hemiplegic migraine. ASL is accessible and sensitive to detecting abnormal perfusion. We should add ASL in pediatric patients presenting with an acute brain attack.
