悪性中皮腫は，胸膜・腹膜・心膜などに存在する中皮細胞から発生するまれな悪性腫瘍で，全体の80％が胸膜，10～20％が腹膜に発生する^1-4）。また肉眼的な発育形式により，限局型，びまん型に分類され，限局型は予後良好であるとの報告がある一方，時間経過でびまん型と同様の進展を示し，連続した発育過程における診断時点の形態分類にすぎないとの報告もされている^5）。

We report a case of sarcomatoid peritoneal mesothelioma, firstly revealed as a solitary localized mass arising from the dorsal aspect of visceral surface of the spleen. 16 months after the first CT, tumor revealed a rapid proliferation and invaded the splenic parenchyma and diaphragm. Several disseminated tumor were also detected. Open biopsy was performed. Comprehensively, the diagnosis was sarcomatoid peritoneal mesothelioma that initially developed in the visceral surface of spleen.
