膵内副脾に発生する類表皮嚢胞は比較的まれな疾患といわれているが，最近の画像診断技術の進歩に伴い報告例は増加している。この疾患は良性の嚢胞性疾患であるが，ときに悪性の嚢胞性疾患との鑑別が難しく，術前に診断がつかないまま手術に至る例も少なくない。そのため術前の画像診断における役割は大きい。今回，我々は非典型的な画像を呈し，診断に苦慮した膵内副脾由来の類表皮嚢胞の1例を経験したので報告する。

A woman in her fifties had a cyst in the tail of her pancreas detected by ultrasonography. The cyst showed high density on computed tomography imaging. On magnetic resonance imaging, the cyst showed hyperintensity at venter and hypointensity at dorsum on T2-weighted images and a mix of hyperintensity and hypointensity on T1-weighted images. Intrapancreatic accessory spleen were not detected. Histopathologic examination showed a cyst covered by fibrous tissue and coated with stratified squamous epithelium including splenic tissue.
