抗リン脂質抗体症候群（antiphospholipid syndrome：APS）とは，抗リン脂質抗体と関連した動脈性または静脈性の血栓症および妊娠合併症と定義され，約半数で全身性エリテマトーデス（systemic lupus erythematosus：SLE）を合併する。今回我々は，副腎梗塞を初発の症状とした若年女性のSLE合併APSの症例を経験した。APSは副腎疾患で発症する場合があり，若年女性の副腎梗塞ではAPSは重要な鑑別疾患となる。若干の文献的考察を加え報告する。

A previously healthy woman in her twenties presented with left flank pain. CT scan showed the left enlarged adrenal gland with no contrast enhancement. It suggested adrenal infarction. Immunological investigations and imaging diagnostic exams confirmed the diagnosis of the APS associated with SLE. In the recent article, the adrenal involvement can be the first manifestation of APS. We concluded APS should be considered in the differential diagnosis for cause of adrenal infarction in young women.
