肺クリプトコッカス症は基礎疾患をもたない健常者，基礎疾患を有する低免疫状態の患者ともに生じ，前者を原発性肺クリプトコッカス症，後者を続発性肺クリプトコッカス症とも称する。画像所見は多彩でありしばしば診断に苦慮する。今回我々は肺クリプトコッカス症を概説し，様々な免疫状態における肺クリプトコッカスの画像所見と可能な場合は病理所見も呈示する。

Though pulmonary cryptococcosis manifests a wide variety of CT findings according to the patients’ immune state and correct diagnosis is sometimes difficult, several predominant characteristic imaging patterns are present. Awareness of these characteristic CT findings, pathological background, and careful attention to the clinical information will allow prompt diagnosis and initiation of early treatment. We should also postulate C. gattii infection in case of atypical clinical course.
