低免疫患者における抗酸菌感染症の画像所見

Imaging findings of mycobacteriosis in immunocompromised patients

Mycobacterium属はグラム陽性細菌に分類される真正細菌の一属であり，いったん染色されると酸・アルコールにより脱色されにくい特徴がみられるために抗酸菌と呼ばれる。日常しばしば遭遇する代表的な抗酸菌はヒト型結核菌とヒト型肺非結核性抗酸菌である。これらによる感染症の画像所見は多彩であり，特に低免疫状態においてしばしば非典型的所見を呈し診断に苦慮する。今回我々は低免疫患者における抗酸菌感染症を概説し，その画像所見を呈示する。

Pulmonary tuberculosis and nontuberculous mycobacteriosis in immunocompromised patients manifest as a wide variety of CT findings according to the patient’s immune state, and correct diagnosis is sometimes difficult. They tend to show atypical imaging findings or primary infection pattern. Awareness of these various CT findings, pathological background, and careful attention to the clinical information will allow prompt diagnosis and initiation of early treatment.
