助産実習における学生のバースレビューによる学習効果—バースレビュー実施後の記録から

Learning Effects of Birth-review by Students in Midwifery Practicum: From data obtained after birth-review

目的

　本研究の目的は、助産実習において学生が実施するバースレビューにどのような学習効果があるのかを明らかにすることである。

対象と方法

　看護大学助産師コースに在籍した学生5名が助産実習で分娩介助を行った産婦のバースレビュー（42件）の『バースレビュー内容』と『アセスメント内容』の記録用紙を用い、記述内容を質的に分析した。本研究は東邦大学倫理審査委員会の承認を得て実施し、倫理的配慮として、学生の科目評価が終了した卒業後にデータ分析を行った。

結果

　学生が褥婦と一緒に実施した『バースレビュー内容』として、7のカテゴリー【分娩プロセスで抱いた思い】、【想起される分娩場面】、【自己の分娩体験の評価】、【我が子への思い】、【家族への感謝】、【学生への評価】、【新たな生活への意思】が導き出された。

　バースレビュー実施内容をもとに学生が記載した『アセスメント内容』における学生の考えや判断として4のカテゴリー【分娩の満足度】、【分娩満足度の根拠】、【褥婦の心理状態の評価】、【育児の視点からの評価】が導き出された。

結論

　助産実習において学生が実施するバースレビューの学習効果として、バースレビューを通して対象や分娩経過に対する理解が促進され、分娩満足に着目した思考が生まれ、褥婦からのフィードバックを得る機会となっていたことが示された。

Objective

The purpose of this study was to elucidate what kinds of effects are obtained through birth-review conducted by midwifery students in the midwifery practicum.

Materials and Methods

A qualitative analysis was conducted on the description in "Birth-review contents" sheets and "Assessment contents" sheets recorded by five midwifery students who carried out birth assistance (n=42) in the midwifery practicum. For ethical considerations, the study was conducted after obtaining approval from the Ethical Committee of Toho University, and the data analysis was conducted only after the students had graduated from the university when a subject evaluation of the students finished.

Results

For the birth-review performed collaboratively with the students and the puerperants, seven categories were extracted; [Thinking conceived in the process of delivery], [Delivery scene recalled], [Assessment of birth experience of own], [Feelings for own baby], [Gratitude for family], [Assessment of students] and [Will for new life].

For students' thinking and judgement seen in the "Assessment contents" sheets described by the students based on the "Birth-review contents", four categories were extracted; [Satisfaction level of delivery], [Reasons for satisfaction level of delivery], [Assessment of puerperants' psychological status] and [Assessment from the childrearing point of view].

Conclusions

As learning effects of the birth-review conducted by the students in the midwifery practicum, it was suggested that 1) an understanding toward puerperants and the delivery process is much enhanced, 2) the thinking focused on satisfactory delivery is created, and 3) it becomes a good opportunity to get feedback from puerperants.
