POSとNANDAの看護診断を用いた高齢者のケアプラン策定

Care Planning for Aged People Using POS and NANDA's Nursing Diagnosis

　医師が総合診療し包括医療をする．ナースは精神的・社会的・経済的情報を収集して，生活像を作成する．ケアワーカーはセルフケアアセスメントを記入して，日常生活動作の状態を明確にする．チーム医療の中でナースがNANDAの看護診断を用いて，PONRの問題リスト（病名や症状や所見にレベルをつける）を作成すれば，その問題リストより高齢者のケアプランが策定できた．看護診断を用いた高齢者のケアプランでは，問題リストをみれば，クライエントの状態が浮かび上がり，ナースとケアワーカーは何をすべきかを知ることができ，誰が策定しても，さほど差がないケアプランが策定できた．

　Care plan for aged people was successfully derived from the problem list of PONR that was made by nurses using NANDA's nursing diagnosis in assessing the chart of general practice and comprehensive medical care filled by doctors in Japanese, the assessment of mental, social and economic status of the clients made by nurses and the assessment of the activity of daily living of the clients made by care workers. Clinical diagnosis, symptom and physical examination of clients was listed in order and the condition of the activity of daily living was classified according to its severity in the problem list. It is easy for nurses and care workers to know what they have to do for their clients because they are able to understand the condition of their clients through the problem list. An international and highly qualified care plan for aged people could be easily obtained using NANDA's nursing diagnosis.
