腹腔鏡下胃内手術における術中偶発症である胃壁貫通部からの出血，血腫形成，誤穿孔を防止するために，腹腔鏡補助下に小開腹を併用して直視下に胃壁を切開し，トロッカーを挿入，縫合閉鎖する方法を考案した．本法は，トロッカーの胃壁刺入，閉鎖に起因する合併症を防ぎ，同時に，トロッカーの挿入位置を病変の局在に応じて正確に選択することを可能とするため，腹腔鏡下胃内手術の適応拡大にも有用であると思われた．
We devised a new technique which was the incision and suture of the gastric wall using the small open laparotomy, during laparoscopic intragastric surgery. Our method prevents intraoperative complications such as bleeding and hematoma of the stomach, or perforation of the intestines. Furthermore, the insertion site of the trocar can be chosen depending on the location of the lesions. The indication of this method might be extend to patients with gastric tumor, irrespective of tumor locations.
