経済性と安全性を考慮した小児腹腔鏡下手術におけるトロッカー挿入

The safety and economical efficiency of the inexpensive trocar insertion with peritoneal anchoring technique in pediatric laparoscopic surgery

◆要旨：目的●小児腹腔鏡下手術のトロッカー挿入において安全性だけでなく経済性も考慮した手技につき後方視的に検討する．方法●安価なトロッカー（リナポート^TM）を腹膜アンカリングを併用し挿入した2005年以降（後期）とそれ以前（前期）でトロッカー挿入時間とトロッカーにかかわる医療材料費について比較検討した．結果●トロッカー1本の平均挿入時間は前期が6.2分，後期が2.1分と挿入法変更後に有意に短縮されていた．医療材料費は5mmトロッカー1本当たり約9,500円のコスト削減が可能であった．結語●トロッカー挿入時の腹膜アンカリングによりトロッカー挿入が容易になり，また安価なトロッカーを使用することが可能になり，医療経費削減が可能であった．

　＜Purpose＞The safety and economical efficiency of the inexpensive trocar insertion with devised technique in the pediatric laparoscopic surgery were reviewer. ＜Methods＞In 2005, the insertion technique of the trocar was modified, adding the peritoneal anchoring stitch at the time of insection. Thirty-four patients were dividet into non-peritoneal anchoring technique group using trocar with safty device（First Group : 8 patients）and peritoneal anchoring technique group（Second Group : 26 patients）using trocar without safty device（LiNa Port^TM, Denmark）. The mean insertion time and the cost of the trocar were compared between both groups. ＜Results＞The insertion time of a trocar was 6.2 minutes in the First Group, and 2.1 minutes in the Second Group. The cost of the trocar was 12,500 yen in the First Group, and 3,000 yen in the Second Group. ＜Conclusion＞The insertion of the trocar became easier, and inexpensive trocar was used safely since 2005 because of the peritoneal anchoring technique. The modification of the insertion technique made it possible to out down the cost in the pediatric laparoscopic surgery.
