◆要旨：当科では2004年よりfingers-assisted laparoscopic surgery（以下，FALS）を基にしたglove fingers assisted laparoscopic surgery（以下，GALS）を導入し，2004年1月～2008年3月にGALSのlaparoscopy-assisted distal gastrectomy（以下，LADG）を122例に施行した．小開腹先行で手術を行い，おもにアリス鉗子，オクトパス鉤を用いて視野展開の手順を定型化した．そのうえでリンパ節郭清の手順を定型化し，安定した術野を維持しての手術の継続が可能となった．GALSにより安定した安全な腹腔鏡下胃切除術が可能になったと考えられる．

　We performed laparoscopy-assisted distal gastrectomy（LADG）with GALS（Glove fingers assisted laparoscopic surgery）procedure based on FALS（fingers assisted laparoscopic surgery）for 122 patients with gastric cancer from January 2004 to March 2008. In GALS procedure, mini laparotomy was made at the beginning of the operation. We standardized the technique of acquiring good operative view by using Allis forceps and Okutopas hook, and thus performed lymph node dissection. We believe GALS contributes to safe and stable procedure in LADG to keep good operative view for gastric cancer.
