直腸切除術における剥離および吻合操作

Dissection and Anastomosis Techniques for Laparoscopic Rectal Resection

　腹腔鏡下手術における直腸の剥離・吻合に関して現時点で可能なこと，解決すべき問題点を明らかにしつつ，実際の手技について概説した．機器の改良・開発と手技の向上により，直腸の剥離・吻合手技の定型化がなされてきた．しかし，上部直腸（Ra）や下部直腸（Rb）の癌に対して，病変への直接把持を避けた腸管の剥離・授動と，適切なsurgical Nmarginを確保した肛門側腸管の切除が機器の制約を受ける部分であり，limiting factorとなっている．狭い骨盤腔内で，良好な術野の下にきわめて繊細な操作ができる腹腔鏡下手術の利点を生かし，操作上の問題点を解決する機器の改良・開発と手技の工夫が今後の課題である．

We described in detail our laparoscopic approach to rectal dissection and colorectal anastomosis. This proce-dure has become standardized due to the developments of instruments and surgical techniques. For deep pelvic dissection, however, the procedure of stapling and retracting while avoiding direct contact with the tumor are often limited. In such cases, laparoscopic procedure will provide better view of the surgical field. With the im-plementation of newer stapling and retracting techniques. we expect rectal dissections with adequate distal margins to become more easily-performed procedure in the future.
