◆要旨：低異型度虫垂粘液性腫瘍（low grade appendiceal mucinous neoplasm：以下，LAMN）は外科的切除が治療の第1選択となるが，術式選択において定まった方針が提示されておらず，本邦でも虫垂切除術からリンパ節郭清を伴う系統学的切除術まで様々な報告が散見される．当院では2018年1月から2022年12月までの期間にリンパ節転移・遠隔転移を有さない8例のLAMNに対して腹腔鏡下盲腸切除術を実施した．年齢は40〜83歳（中央値76歳）で，男性2例，女性6例であった．術後在院日数は4〜20日（中央値：5.5日）で，いずれも術後合併症を認めず，2023年1月現在で無再発生存を継続している．腫瘍境界の評価が十分に行われた症例では，腹腔鏡下盲腸切除術で過不足なく，安全な切除が達成できると考えられる．
Surgical resection is the first-line treatment for low grade appendiceal mucinous neoplasm（LAMN）, but there is no established policy for surgical strategy, and there are various reports ranging from appendectomy to systematic resection with lymph node dissection. We performed laparoscopic cecectomy for 8 patients with LAMN without lymph node or distant metastasis from January 2018 to October 2022. The age ranged from 40 to 83 years（median : 76 years）, with 2 males and 6 females. The postoperative hospital stay ranged from 4 to 20 days（median : 5.5 days）, and none of the patients had postoperative complications and remained recurrence-free at January 2023. In cases where tumor boundaries have been adequately evaluated, laparoscopic cecectomy can achieve safe resection without excesses or deficiencies.
