要旨●虫垂癌は希少癌腫ではあるが，虫垂炎の切除後病理診断という形で一般病院でも遭遇する．治療方針に関する本邦のガイドラインは存在しないが，欧米では参考となる文献がある．大腸癌研究会ではプロジェクト研究として全国の虫垂腫瘍切除例の集積を行った．全国43施設より2000〜2017年の間の922例が登録された．これらの症例の予後はUICC/AJCCのTNM分類第8版による病理ステージングを反映していた．UICC/AJCCでは低異型度虫垂粘液性腫瘍（LAMN）を悪性腫瘍として取り扱っており，本邦でも今後癌登録されることで，予後追跡が全国的に行われることを期待したい．

　Appendiceal cancer is rare, but it is encountered in general hospitals as a pathological diagnosis after appendicitis resection. In Japan, guidelines regarding treatment policy remain unavailable, but some related literature has been published in Europe and the United States. Japanese Society for Cancer of the Colon and Rectum has collected data on appendiceal tumor resection cases nationwide as part of a project study. A total of 922 cases were registered from 43 facilities nationwide from 2000 to 2017. The prognosis of these cases reflected the pathological staging following the eighth edition of the Union for International Cancer Control/American Joint Committee on Cancer（UICC/AJCC）tumor, node, and metastasis classification. The UICC/AJCC considers low-grade appendiceal mucinous neoplasm（LAMN）as malignant tumors. We hope that cancer registration will be established and prognosis follow-up will be conducted nationwide in Japan.
