◆要旨：【目的】高度肥満症に対する腹腔鏡下スリーブ状胃切除術におけるVeress needle（気腹針）による第一トロッカー挿入の安全性について検討する．【方法】当院にて行われた同手術について先行気腹の成功率，気腹針に起因する合併症の発生率などについて検討した．【結果】2014年6月から2020年9月までに138例に同手術を施行した．執刀医として11名の外科医が経験者の指導の下で気腹針挿入を行った．先行気腹の成功率は99.3％（137/138例）であった．気腹針に起因する損傷などの合併症はみられなかった．【結論】気腹針を用いた高度肥満症に対する腹腔鏡下スリーブ状胃切除術は，経験者の指導の下では安全性が高く安定した手術手技であると考えられた．
［Objective］ To investigate the safety of initial pneumoperitoneum created with a Veress needle in laparoscopic sleeve gastrectomy for morbid obesity. Methods: The present study investigated the success rate of prior pneumoperitoneum and the incidence of pneumoperitoneum-related complications in laparoscopic sleeve gastrectomy performed at our hospital. ［Results］ One hundred thirty-eight patients underwent the procedure between June 2014 and September 2020. Eleven surgeons performed needle insertion under the supervision of an experienced colleague. The success rate of prior pneumoperitoneum was 99.3％ （137/138 cases）. No complications, such as damage caused by needle insertion, were observed. ［Conclusion］ Laparoscopic sleeve gastrectomy for morbid obesity using a Veress needle was safe and readily feasible if performed with the guidance of an experienced surgeon.
