腹腔鏡下鼠径ヘルニア修復術後のメッシュ感染に対して腹腔鏡下メッシュ除去術を施行した1例

Laparoscopic mesh removal for postoperative mesh infection after laparoscopic transabdominal preperitoneal inguinal hernia repair

◆要旨：症例は83歳，男性.両側鼠径ヘルニアに対して腹腔鏡下鼠径ヘルニア修復術（transabdominal preperitoneal repair：以下，TAPP法）を施行し，7か月後に右鼠径部痛が出現した．腹部造影CT検査で右鼠径部から腹膜前腔に長径55mmの膿瘍を認め，メッシュ感染と診断し，腹腔鏡下メッシュ除去術を施行した．術後10か月が経過したが，感染の再燃やヘルニアの再発を認めていない．今回われわれはTAPP法施行後のメッシュ感染に対して腹腔鏡下メッシュ除去術を施行し，良好な経過を得られた1例を経験したので報告する．

　A 83-year-old man who had undergone laparoscopic transabdominal preperitoneal repair （TAPP） for bilateral inguinal hernia presented with right inguinal pain7 months after the surgery. Computed tomography revealed abscess formation in the right inguinal region. Postoperative infection was diagnosed, and laparoscopic mesh removal was performed. Ten months later, no recurrence of infection or hernia was observed. The present case of post-TAPP mesh infection was successfully treated using laparoscopic mesh removal.
