15．免疫関連有害事象（irAE）—新種のオンコロジック・エマージェンシーに，ICUで適切かつ果敢に立ち向かうために

免疫チェックポイント阻害薬（ICI）は癌治療の新しい柱として登場し，瞬く間にその変革的な治療成績と地位を確立しつつある。一方，治療過程で自己免疫疾患類似の全身性炎症性副作用が生じることも明らかになってきた。この副作用を免疫関連有害事象（irAE）と呼び，高頻度に出現するのは皮膚毒性ならびに消化管毒性，致死率が高いのは心毒性ならびに神経毒性であることがわかっている。いずれの病型も，短期間に悪化し生命を脅かし得る。本稿は，各irAEの病型について，どのような場合にICUで遭遇する可能性があるのか，癌治療科の主治医と集中治療医がどのように連携をとることが望ましいかを語るうえで前提となる，irAEの必須事項を理解する助けとなることを目指す。

Immune checkpoint inhibitors （ICIs） have emerged as a new pillar of cancer treatment and are quickly establishing themselves with their revolutionary therapeutic results, but it has become clear that systemic inflammatory side effects resembling autoimmune diseases occur during the treatment process. These adverse reactions are referred to as immune-related adverse events （irAEs）, with cutaneous and gastrointestinal toxicities being the most frequent, while cardiotoxicity and neurotoxicity are the most fatal. This article aims to help readers understand the essential basics of irAEs, which are required for understanding how each type of irAE may be encountered in the intensive-care unit and how oncologists and intensivists should work together.
