【コラム2】口腔ケア—口腔ケアはVAPを予防できるのか？

口腔ケアは日常生活援助であるとともに，主に人工呼吸器関連肺炎（VAP）予防を目的として行われる。口腔ケアに関して，クロルヘキシジンを使用するか否か，頻度，ブラッシングをするか否かなど，多くの議論がなされてきた。クロルヘキシジンに関しては，人工呼吸器ケアバンドルを含め，各国のガイドラインで推奨されてきた歴史がある。しかしながら，近年，その効果や有害事象を再評価する気運が高まっている。クロルヘキシジン以外の洗口液に関しては研究が少なく，結論は導き出せない。ブラッシングに関しては一定の効果が認められており，推奨できるかもしれない。口腔ケアの頻度や体位に関しては比較試験がほとんどなく，結論を出すことは困難である。近年では，口腔内の評価と，それに基づいた介入が重要視されており，今後，これらのプロトコルの確立と導入が重要になるだろう。

Oral care supports daily living and is primarily used to prevent ventilator-associated pneumonia. However, there has been controversy regarding the frequency of oral care, whether or not brushing should be used, and whether or not chlorhexidine should be used. Chlorhexidine has a long history of recommendations, including the ventilator bundle. In recent years, however, its efficacy and adverse events have been attracting attention. There are few studies on mouthwashes other than those containing chlorhexidine, and definitive conclusions cannot be made. As for brushing, certain effects have been observed and may be recommended. Few comparative studies are available regarding the frequency of oral care and cleaning methods, which makes it difficult to draw conclusions. In recent years, oral assessment and interventions based on these assessments have become more important. These protocols must be established and implemented in the future.
